One key to keeping a project on track is to maintain careful records of change requests. To help with that process, project management expert Tom Mochal created this scope change request log, which enables you to consolidate and summarize all the requests into one easy-to-read document. The log is designed to hold essential but streamlined information, including a brief description of the change and who requested it, the date that the request came in, and when it was resolved. You can also specify the priority of each item, its status, and any comments or notes related to the resolution of the request.

We've updated the log in PDF format to make it easy to use as a printed document; we also created an Excel workbook version for users who would rather maintain the log electronically.
